ANSWER

Digoxin for pediatrics.

     * Mechanism of action.

             * It inhibit sodium pump (Na+-K+-ATPase) which leads to

               influx of calcium by a sodium and calcium exchange mechanism.

             *It also reduces the aldosterone,norepinephrine and renin levels and

               and stimulate diuresis.

             * Parasympathetic system activated by the action of digoxin which slows

               conduction of impulses through AV node.

    * Therapeutic use.

            Used in treatment of

                             * Atrial fibrillation.

                             * Atrial flutter.

                             * Congestive heart failure.

     * Complications.

                        * Cardiac dysrythmias. (Bradycardia).

                        * Hypokalemia.

                        * Dizziness.

                        * Heart block.

                        * Blurried vision.

   * Contraindications / precautions.

                     *Digoxin is not used in patients with

                                * Sinus bradycardia.

                                * Cardiomyopathy.

                                * Cor pulmonale.

                                * Hypercalcemia.

                                * Myocardial infarction.

                                * Hypomagnesemia.

                                * Renal failure.

* Interactions.

             Drugs that enhance digoxin toxicity.

                        * Diuretics

                        * Calcium.

                        * Verapamil.

                        * Methyldopa.

         Drugs that reduce digoxin level.

                        * Neomycin.

                        * Antacids.

                        * Metachlopromide.

                        * Phenobarbiturate.

                        * Rifampicin.

   * Medication administration.

                  Pediatric dose.

                            Tablet - 0.125 mg, 0.25 mg.

                     Oral solution - 0.05 mg/mL.

                     Injectable solution - 0.1 mg/mL,0.25 mg/mL.

* Nursing interventions.

               * Nurse should check electrolyte level of a patient who undergo digoxin

                 therapy to avoid digoxin toxicity.

               * Assess the heart rate of the patient and do not give administer digoxin if

                 the patient has bradycardia.

               * Continous monitoring needed to identify any signs of dysrhythmia during

                  treatment.

                * If digitalis toxicity occur stop digitalis infusion and give parenteral or oral

                   pottasium supplements and digibind (Antidote) to overcome the toxicity.

                * In the case of severe toxicity begin antidigoxin immunotherapy as per

                   physicians order.

* Client education.

                * Educate the care givers of child about abnormalities may occur during

                   digoxin therapy like changes in heart rate and rhythm and if any changes

                   observed inform the health care provider immediately and stop giving next

                    dose of the drug.

*Evaluation of medications effectiveness.

                   * Effect of digoxin therapy can evaluate by

                              * Assess the heart rate and rhythm of the patient after digoxin
