ANSWER :

Expected Pharmacological Action

Clotrimazole is an antifungal medication. It is an imidazole derivative which works by inhibiting the growth of individual candida or fungal cells by altering the permiability of fungal cell wall.It binds to phospholipids in the cell membrane and inhibit the biosynthesis of ergosterol and other sterols required for cell membrane production.

   Therapeutic use :

  • used to treat vaginal infections
  • oral thrush
  • diaper rash
  • pityriasis versicolor
  • athlets foot
  • jock itch

Complication :

  • Oral intake leads to itching, nausea, vomiting, abnormal liver function test ( more than 10% of patients ).
  • When used to treat vaginal yeast infection, less than 10% patient have vulvar or vaginal burning sensation.
  • Less than 1 % of patient have polyuria, vulvar itching, edema, discharge.
  • Clotrimazole creams and suppositories contain oil which may weaken condomns and diaphragms.

Medication Administration :

The routs of administrations are throgh mouth, applied as a cream to skin, locally in the vagina.

Contraindication / Precaution

  • Known case of drug allergy
  • First trimester of pregnency
  • known case of chronic liver disease
  • Self administration of clotrimazole more than one week
  • Avoid exposure to eyes.

Interactions :

  • No drug interaction with topical tropical clotrimazole .
  • Clotrimazole is CYP3A4. enzyme inhibitor.So any medication that is metabolized by the CYP3A4 enzyme ( acetaminophen, codien, diazepam, erythromycin )will have elevated levels when oral clotrimazole is used. So this medication should be avoided with oral clotrimazole medication.​​​​​​​

Nursing Interventions :

  • Advise patient to report increased skin irritation.
  • usual oral dose of oral clotrimazole 10 mg 5 times per day for 14 days.
  • nurse should making that patient is taking adequate dosage of drugs for adequate duration.
  • check for five rights like right patient, right drug, right dose,right route and right time.

Evaluation of medication effectiveness:

  • Ask to patients about the improvement of symptoms like itching, discharge etc...
  • look for the size of leision and compaire with the leision at the start of treatment.

Client Education:

  • Patient must be educated about the correct dose, frequency, duration of treatment.
  • Oral medication should not be swallowed and allow to dissolve in mouth about 10 to 15 minutes.
  • The medication should start working in first week. Apply the oinment at least 14 days after the infection is disappear, as it takes a while to kill all the fungus.
  • ​​​​​​​Periodically check the liver function test and report any advise events.
