Ans) Misoprostol
Expected pharmacological action:
- Act as a prostaglandin analogue, decreasing gastric acid secretion (antisecretory effect) and increasing the production of protective mucus (cytoprotective effect).Causes uterine contractions.
Therapeutic use:
- Misoprostol helps to decrease your risk of serious ulcer complications such as bleeding. This medication protects your stomach lining by lowering the amount of acid that comes in contact with it. This medication is also used in combination with another drug (mifepristone) to end a pregnancy (abortion).
Contraindication:
- Contraindications include pelvic infection or sepsis, hemodynamic instability or shock, allergy to misoprostol, known bleeding disorder, concurrent anticoagulant therapy, and confirmed or suspected ectopic or molar pregnancy.
Medication administration:
PO: (Adults) Antiulcer200 mcg 4 times daily with or after meals and at bedtime, or 400 mcg twice daily, with the last dose at bedtime. If intolerance occurs, dose may be decreased to 100 mcg 4 times daily. Termination of pregnancy400 mcg single dose 2 days after mifepristone if abortion has not occurred.
Nursing Intervention:
- PO: Administer medication with meals and at bedtime to reduce severity of diar- rhea. Antacids may be administered before or after misoprostol for relief of pain. Avoid those containing magnesium, because of increased diarrhea with misoprostol.

Client Education:
- Misoprostol is usually taken with meals and at bedtime. Follow your doctor's instructions. You may have nausea, stomach cramps, or diarrhea while taking this medicine, especially during the first few weeks after you start taking misoprostol. These symptoms usually last for about a week.
Evaluation of Medication effectiveness:
