Each patient needs to be assigned a chart number as a chart number is used for linking together all the information regarding a patient that is maintained in the different databases, for example the name of the patient, address, charges, and insurance claims. Thus all the documents are tagged with the chart number as it is the key to the filing system.

It is necessary to create a new case in Medisoft when a patient is seen for more than one case i.e. disease condition. Thus when a new health issue presents itself, a new case has to be created in Medisoft.
