Patient safety should be made the top priority in all hospitals. The health care teams should reduce the infection rates, put checks in place to prevent mistakes and to ensure strong lines of communication between hospital staff, patients and family. Some of the causes f errors are: human factors- variations in training, experience, fatigue, depression, increase working hours; medical complexity- complicated technology, powerful drugs, intensive care, prolonged hospital stay; system failures- poor communication, increase in patient nurse ratio, drugs that look and sound alike, environment and design factors and infrastructure failure. Some of the patient safety tips for hospitals are: prevention of central line associated blood stream infection , reduction in the readmissions, prevention of venous thromboembolism, educating patients regarding the use of blood thinners, limit the shift timing, use a good hospital design, assess the hospital patient safety, cultivate effective health teams, have a good rapid communication between the health team members, perform medical procedures safely.
