Physical assessment

Nothing significant other than the sleep disturbance

Mental status examination

It reveals the person appeared to be anxious with depressive cognitions, like I feel nothing.

In thought he had the intrusive thoughts about the flash backs about Iraq about the IED blasts. With a good insight and orientation.

The medical lab result everything comes with in normal with negative for all the substance in the blood test.

So, this case can be formulated as a person with PTSD having significant sleep disturbance and problems in thought about the event and being presented with an anxious face with no delusion or hallucination but a clear-cut history of depressive cognition with suicidal ideation and don’t having nay active thoughts. In the past use of alcohol and abstinent for about 1.5 years.
