A.C.T.I.O.N means:

A- assess or observe the situation.

C- Contact the supervisor

T- talk to family member if present

I - identify how we can help the patient or client

O- Open the chart and document the problem.

N- note to follow up the patient.

It is a resource guide provided to home workers when they encounter medication related problems.

each section of this screening tool and resource guide is colour coded and if the problem is identified on the screening tool then one must go to the corresponding colour section of the resource guide and start A.C.T.I.O.N plan

The following are the different sections of the resource guide with their respective colour

SECTION A ( red colour)

it includes problems associated with taking medications correctly like dose, frequency, mode of administration , time of taking .

SECTION B (Blue colour)

it includes physical or cognitive problems of the patient

SECTION C (Violet colour)

it includes barriers like the patient is unwilling to take the medications or he/she has expired medications

SECTION D ( yellow colour)

it includes changes in patients condition like changes in behaviour ,mood or bowel patterns etc

SECTION E (green colour)

it includes changes in patients medications like change in dose or frequency.
