The purpose of the tool is to guide when a medication related error or problem is identified where A stands for assess/ observe the situation,C for contact the supervisor, T stands for talk to family member if present,I means identify how you can help,O for open the chart and document and N for note to follow up.

In situation 1.
a. problem identified is that client needs more caregiver support with medication.

b. Section c will be referred i e barriers and other problems

c. Action in this case will be ASSESS/OBSERVE the situation Does the client have a reliable caregiver to help with medications? Was there a previous caregiver who is no longer involved? NO CONTACT supervisor and inform him/her that the client does not have a reliable caregiver to help with medications TALK to family member if present IDENTIFY how you can help OPEN the chart and document what you have done NOTE to follow-up next visit to see if client is still having problems

in situation 2

a. Problem identified is that client has difficulty taking or preparing medication as he has rheumatoid arthritis.

b. Section b will be referred that is physical and cognitive problems

c. ASSESS/OBSERVE the situation Is the client having difficulty with stiffness, painful hands, or shakiness? Is this difficulty related to clumsiness or weakness? Is there another problem that is preventing the client from opening his/her medication bottles? CONTACT supervisor and inform him/her that the client is having difficulty opening medication bottles TALK to family member if present IDENTIFY how you can help Follow agency policy on medication procedures OPEN the chart and document what you have done NOTE to follow-up next visit to see if client is still having problems

in situation 3

a. client is unwilling to take medication as prescribed

b. Section c barriers and other problems

c. ASSESS/OBSERVE the situation SECTION C Ask the client why he/she is not willing to take medication Why does the client feel this way? CONTACT supervisor and inform him/her that the client is unwilling to take medication TALK to family member if present IDENTIFY how you can help Follow agency policy on medication procedures OPEN the chart and document what you have done NOTE to follow-up next visit to see if client is still having problems Medication Problem
