c. What action would you take to assist your client? Record your answer for part as if you are documenting in the client's record. (3 marks for each situation)
Situations
.1. Your client has Parkinson's disease. He tries to administer his own oral medication. His wife, who usually helps him, is often not home for his afternoon dose because your visit is her respite time.
2. Your client is 84 and has rheumatoid arthritis. Each week the RN distributes her medication for each day in a docette. Today your client's joints seem very inflamed and she asks you to open her docette for her. 3. According to the care plan for a new client, he is on an anti-inflammatory medication that has been prescribed to be taken bid. When you try to remind him to take his medication, he refuses, claiming he is fine. 4. While reading the label on your client's medication, you notice that the strength of the medication is different from the care plan. Your client claims that his meds were changed over the weekend after visiting the medical clinic.
Situation 1
A) In this case the client has parkinson's disease (C.N.S disorder causes tremors,stiffness) so it is difficult for him to find his medicines and take them .His wife is often not home to give him his afternoon dose so there is a chance to miss the afternoon dose or even if he takes there is chance of taking wrong medication or wrong dose.
B) This issue can be addressed by going to section B(physical and cognitive problems) and section C( Barriers and other problems) and then apply A.CT.I.O.N plan
C) I would talk to his wife regarding the afternoon medication and dose and i would help the client to take his afternoon medication when his wife goes out.
SITUATION 2
A)In this case, the client has rheumatoid arthritis( auto immune disease which affects the joints) and he is taking medicines for that but now the joints are very inflammed suggesting that there is change in his health condition and requires consultation with the physician and so there is a issue whether to continue the same old medications or not.
B)This issue can be addressed by going to section D ( changes in client condition)
C) i would talk to the supervisor regarding the issue and also explains the problem to the client's attendant as well if present.
SITUATION 3
A)In this case, the client was being prescribed anti inflammatory drugs two times a day which he is refusing to take claiming that he is fine but the issue is is he really fine or is it just an excuse to avoid the medication . if it is an excuse his problem for which the anti inflammatory drugs are prescribed will worsen.
B) This issue can be addressed by going to section C (Barriers and other problems)
C) I would talk to the patient first regarding why he does not want to take the medications and also ask him if he is facing any problem after taking the medications. Then i would inform the issue to the supervisor and to the client's family member as well.
SITUATION 4
A)In this case, the strength of the medication is not matched with the care plan and i was not aware of the fact that the medications were changed after a recent visit to the clinic as claimed by the client but the latest care plan is not available so there is a doubt regarding the right strength to be administered.
B) This issue can be addressed by going to section A ( Problems taking the medications correctly)
C) I would inform this issue to the supervisor and seeks help and i also talk to the client's family member and verifies the issue.
Situation-2
a. Rheumatoid arthritis causes joint pain and swelling, reduced
mobility and physical weakness. General tiredness, trouble sleeping and exhaustion are other common symptoms.
b. This issue will be addressed by going to Section-D (Changes in client condition). According to care plan the
supervisor will be informed about the change in health condition.
c. The joints are now very inflamed suggesting that there is change in her health condition and requires consultation with the physician and so there is a issue whether to continue the same old medication or not. I would talk to the supervisor regarding the issue and also explain the problem to the client’s RN as well if present and make a note to document the change in health condition.
