Home Assignment Case Study The background You are a member of a service redesign group who have been asked to plan changes in services for people with long term conditions and with evaluating changes. Much health and social service use is by people with chronic disease and who are age 65 and over. Reducing multiple admissions to acute hospitals is a key aim of service innovation, with attempts to substitute effective and efficient community health care systems as alternatives. Medical/social care of people age 65 and older with multiple chronic conditions (mainly heart disease (HD), stroke and chronic obstructive pulmonary disease (COPD), especially in the last year of life) is one of the most prominent concerns for health care systems all over the world. It is important not only because of size and cost, but also quality and appropriateness of care are difficult to achieve. You will be using information describing a recent service innovation project in Brent, North West London, which was designed to change the health care use patterns affecting many people with long term conditions, many with co-morbidities. A key innovation was developing an integrated assessment and referral service (called Integrated Care Co-ordination Service (ICCS) in the community to obviate the need for hospital admissions, mainly readmissions. This Brent project has been selected by the Department of Health as a key innovation and will be rolled out to other places.

  • The Integrated Care Coordination Service (ICCS) in Brent, north London, main aim is be supportive to older people at risk of admission to hospital or residential care. The necessities of those older people were assessed by a care coordinator, who has the ability to provide vast range of essential services from agencies .The important aim of Brent project is to connect  Rapid Response (RR) and Whole Systems Integrated Care (WSIC) models of care. (The Whole Systems Integrated Care models have set of software tools which make clinicians and care professionals who are providing direct care to patients ,available. Rapid response (RR) care service model provides specialist, coordinated, comprehensive and supportive assessment and treatment 24 hours a day in the patient's own home). So brent project tries to provide quality care at rapid rate with efficient excellent clinicians and professionals throughout a day in their own homes.
  • The outcomes of brent project are it serves as benefit and means of saving which came to the health service, and we are this may help social services too in the longer run by enabling people to stay longer in the community.
  • BRENT PROJECT provide ample support during crisis and also serves as many patients throughout their illness from crisis state till recovery phase and also even after that , if the client needs it.
  • It also aims to reduce the demand for unnecessary urgent and emergency care.
  • Brent project is patient oriented and also a co ordinated care for patients with easy access to high quality with appropriate timing and deliver sumptuous care for elderly .Overall it delivers holistic care.
  • When a planned care is available for 24 hours a day, 7 days a week, 365 days a year with highly professional healthcare providers near home will definitely decrease the rates of admission and also the rate of disease progression may also be slower. It serves as a preventive care and aims towards better health.
