Absence of blame is very much essential to the effectiveness of creating a culture of safety as it builds up a feeling of trust between the fellow people and as well as between the patient and health consultant.

The veterans administration emphasizes on prevention rather than punishment, because they feel like you can not do anything if damage is done and moreover punishment can do more damage than handling the things. It will make frustrated staff make matters worse. Nurse leaders can facilitate this culture change but learning through the mistakes and try to prevent them.
