Decision making is one of the major skill of nursing profession. A sound decision making and problem solving approach at the right time makes the nursing personnel to provide the individualised care of approach. To make the proper decision data is need the data can should be collected, organised, validated and documented appropriately. When prioritising the problems and needs of the patient the data should be double checked to go into a decision making so that the treatment or the intervention would be more effective.

The patients problems change everyday and it has to be looked into and when judging the interventions based on the current scenario the quality can be increased with proper assessment and data collection to improve the change process often the nurse must collect both the subjective and objective data through available primary and secondary sources.

This makes the nurse to prioritise problems into actual, risk or wellness which would help her to set the goal or desired outcome in order to provide quality and better miliu environment free from risk of injury or threat..
