Nurses plays a vital role in the care of patients. In order to provide a continuous care by the staffs the nursing care plan is very essential.The nursing care plan consists of the following

Assessment:It is categorized into subjective data (comprises of complaints or problems ,needs ,etc., verbalized by the patient) and objective data (comprises of the signs or the observations made on the patient by the healthcare professionals)

Nursing Diagnosis : is a mixture of three components in general giving a complete knowledge on the patients conditions and what intervention is needed for the patient

eg., Acute pain (in abdomen)/ related to inflammation of the gastric layers (gastritis)/ as evidenced verbalisation of pain ,pain score,facial grimace, lack of participation

In simple the chief complsint/why the complaint occured / and how it was identified

Outcome/Goal

This consists of relieving the patient complaints

(The patient is able to do all activities and has no pain)

Nursing intervention

This comprises of what action can be done in order to provide relief and comfort or alleviate the symptoms

(Administer analas per order)

Rationale

This consists of medical rationality behind each nursing intervention

(Administration of analgesics can reduce pain)

Evaluation

Assess the patient finally in regards to prognosis (progress or detoriation ) in regards to the care provided

(The pain level is decreased )
