Peptic ulcer develop in an acid environment. Acids, bile salts, aspirin, NSAIDs, alcohol, ischemia, H. pylori all these factors cause breakdown of gastric mucosal barrier. The back diffusion of HCL acid into the gastric mucosa results in cellular destruction and inflammation. Therfore acid secretion and pepsin release occurs. Histamine releases from damaged mucosa increasing vasodilatation and capillary permeability. Acid and pepsin causes further mucosal erosion, destruction of blood vessels and bleeding. And ulceration occurs.
Affecting body after peptic ulcer is common as it causes burning or gaseous pain in high left epigastrium, back and upper abdomen, occasional nausea and vomiting
