Dehydration is defined as the significant loss of body fluid that impairs body functions. Dehydration assessment includes the following parameters and it's sub categories :-

1. General condition :-

- well alert

- restless ,irritable

- drowsy ,lethargic ,cold extremities

2. Eyes :-

- normal

- sunken

- very sunken ,dry

3. Anterior fontanelle in infants and children :-

- normal

- depressed

- very depressed

4. Tear :-

- present

- absent

5. Thirst :-

- mild

- moderte

- excessively thirsty

6. Mouth and tongue :-

- moist

- sticky

- dry

7. Skin turgor:-

- slightly decreased

- decreased

- very decreased

8. Pulse :-

- slighlty inrease

- rapid and weak

- rapid ,some time impalpable

9. Blood pressure:-

- normal

- decreased

- hypotensive shock

10. Respiratory rate :-

- slightly increased

- increased

- deep ,rapid

11. Urine output :-

- normal

- reduced

- markedly reduced
