Cholinergic agents activate the cholinergic receptors of the smooth muscle in the urinary bladder leading to an increased peristalsis thereby treating urinary retention due to a neurogenic bladder.

Anticholinergic drugs inhibit the binding of acetylcholine to its cholinergic receptor, suppressing involuntary bladder contraction leading to an increase the volume of the first involuntary bladder contraction, decreasing the amplitude of the involuntary bladder contraction, thus causing an increase bladder capacity.
