1. Patient education and discharge from for Renal failure :-

Monitor fluid and electrolyte balance

Weight regularly

Promptly replace fluid losses

Adherence to medications

Monitor renal function in patients taking potentially nephrotoxic drugs

Caution patients regarding the use of NSAIDs

Avoid decreased perfusion pressure and prevent hyperkalemia by avoiding ACE inhibitors.

Assess signs and symptoms of hypervolemia (oliguric phase) or hypovolemia (diuretic phase), as well as electrolyte imbalances

Prevent infection

Perform skin care and take measures to prevent pressure ulcers

Prevent stomatitis through effective mouth care

Monitor patient for signs and symptoms of drug toxicity

Regular follow up

Explaining about the warning signs and when to reach the physician
