1.Risk for decreased cardiac output related to fluid imbalance and accumulation of toxins.

2.Risk for ineffective protection related to decreased RBC production

3.Risk for impaired skin integrity related to accumulation of toxins and edema

Outcome

1.Maintain cardiac output as evidenced by BP and HR within patient 's normal range.

2.Patient has no evidence of bleeding.

3.maintain intact skin

Intervention

  • Assess heart and lung sound
  • Monitor edema
  • Monitor BP, heart ratt
  • Assess activity level
  • Promote bed rest
  • Administer antihypertensive as prescribed
  • Limit fluid intake
  • Prepare for dialysis
  • Assess the skin
  • Assess bleeding
  • Avoid invasive procedure
  • Monitor RBC, PT INR, BUN, serum urea and creatinine
  • Administer fresh blood, packed RBCs as indicated.
  • Administration of erythropoietin as prescribed
