Tara is a 37-year-old African American married multigravida. She is 8 weeks pregnant and has come to the women's health clinic for her first prenatal visit. Her first two pregnancies resulted in spontaneous abortions at 13 and 14 weeks. Her third pregnancy resulted in the birth of her 4-year-old daughter at 36 weeks of gestation. Tara smokes one pack of cigarettes every 1 to 2 days. Her health history reveals that she has been hypertensive since age 20 and often experiences bladder infections. Tara is a first-grade teacher at a private school. She tells the nurse that this school year has been especially stressful, since her class size was increased with the combination of three first-grade classes into two. Tara explains that she needs to be on her feet most of the day keeping the children busy and attentive, and when she gets home her 4-year-old daughter keeps her busy until bedtime at 7 PM. Tara's husband tries to help, but he often gets home late and occasionally has to be out of town on business.

Preterm labor is one in which the labor pain starts early, before 37 weeks.
The focus of the management is to prevent the premature onset of labor, arrest preterm labor, proper management of labor and effective neonatal care. It happens that majority of the reason of cause remains unknown and hence decision need to be taken whether to terminate the pregnancy or continue. The primary care is to reduce the occurrence of preterm labor by reducing the high risk factors, secondary care which includes screening test for early detection and prophylactic treatment and tertiary care should be focused to reduce the perinatal mortality and morbidity. Investigation includes full blood count, Urinanslysis, USS, serum electtolytes and glucose levels.
