7) Nursing care plan :-
Nursing diagnosis - risk for infection related to peripheral line.
Nursing interventions - Assess the IV site for redness , inflammation , check the temperature ,any drainage etc .
- use of aseptic technique while providing care to the patient during administration of medications ,IV fluids etc
- hand washing before and after administration of medications and injection
- changing the dressing every 72 hours
- keeping the area dry and clean
- if the patient has discomfort in the peripheral line ,check the line for any emboli or clot etc, discontinue the fluids and medicine and use a new line .
- flush the peripheral line before and after administration of medication.
8. The most important thing the nurses missed is assessing the comfort of the patient in the sling because if he is not comfortable then there are higher chances of getting injured during shifting .
9. Nutrition affects skin integrity because the vital nutrients like :-
Protein helps to maintain the skin , repair minor defects , and preserve intravascular volume.
Vitamins C/ zinc / copper helps in the formation of collagen .
Hydration is very important to maintain turgor . Dehydration leads to poor turgor .
So, if we don't take a balanced diet it will affect our skin integrity and cause impairment like pressure sores etc .
15. The nurse should use a therapeutic relationship with the patient and should encourage the patient to express his concerns , anxiety etc . Also the nurse should explain in detail about the procedure including preparation ,side effects , complications etc in a language which is understood by the patient and then take the informed consent .
