The steps of the case management processes are divided as follows

  1. Screening
  2. Assessing
  3. Evaluating risk factors
  4. Planning
  5. Implementing
  6. Following up
  7. Evaluating outcomes

Screening:

Need to screen the cases because to decide the case needs process. The screening helps to avoid unnecessary time spending on resolving the smaller problems.

Assessing:

a complete study and understanding of the case to resolve the problem is required. The manager talks with the client and collects the information related to the case for assessing the case problem.

Evaluating risk factors:

once the case is assessed, then the case is evaluated for any risks to prioritize the case. The manager also decides how much intervention is required to resolve the case.
