Nursing Diagnosis:::Risk for infection related to low defense mechanism of an individual

Goals and outcomes:::1)patient get free from infection

2)prevent infection by early findings

3)family members  understand about hand washing techniques

Nursing interventions:::1)Teach parents aseptic techniques for dressing changes,would care,peripheral care,and cathetercare..tell them to wash hands properly before and after touching patient,body fluids,before giving feed etc.

2)encourage the parents to give Protein and calorie rich diet.

3)Tell the parents to change the position of child frequently.

4)limit visitors and give surgical mask to visitors to prevent respiratory infections.

5)tell them to avoid contact with patients who have infectious disease as children are more prone to get disease easily.

Evaluvation:::1)risk of infection get decreased as parents follow all instructions provide by health care worker to avoid infection.
