Nursing care plan. 1:

Assessment :

Subjective data : client has been complaint about lam, having swelling lam unable to bare that.

Objective data : edema

Nursing diagnosis :

Fluid volume excess related to decreased cardiac output as evidenced by edema.

Goal or expected out come :

After implementation of nursing interventions the client may be reduces edema.

Interventions and rationale :

* assess the presence of medical condition that will be potentiate fluid volume excess.

By these we can know the precipitating factor of edema.

* monitor the client weight and compare weight from date of admission to now.

So that we can know the improvement of client fluid accumulation status.

* assess the patient weight daily, and maintain intake and out put chart,

So that we can know the positive or negative balance of fluids.

* elevation the foot end, so that we can reduces edema.

* apply crape bandages, it will reduces swelling of the legs.

* educate regarding fluids restriction, and salt restriction diet so that clients get an idea how to get reduces edema.

* administered diuretics, like lasix 20 mg bd, dose may increases the urine out put and useful to reduce edema.

Evaluation :

Client may reduced from edema and weight also decreased.

Assessment :

Subjective data :

Client has been complaint about iam feeling over weight and I'm unable to do my work actively.

Objective data :

Obesity, or excess body weight known by client BMI is 31.4 kg per meter square.

Nursing diagnosis :

Imbalanced nutrition more than body requirements related to excessive intake of fat diet as evidenced by client BMI is more than 30 kg per meter square.

Goal or expected out come :

Client may get weight Loss after implementation of nursing interventions.

Interventions and rationale :

* assessment of client body weight, daily, it will give baseline data to plan nursing interventions.

* Educate the client regarding excess fiber diet and low fat diet. Which may useful to reduce body weight of the client.

* educate regarding regular exercises like walking, jogging ect. It may burns the calories and extra fat may reduces.

* educate the client regarding daily dietary menu plan according to needed nutrientional values ,so that we can cut shot the extra nutrientional values.

* educat client to take more grean leafy vegetables and fruits. To digest fats.

Evaluation :

Client reduced from over weight , that is from 160.8 pounds to 158 pounds.

Assessment :

Subjective data:

Client has been complaints about she is not able to get remembering the things, and forgetting, what happens.

Objective data :

Impaired memory or dementia.

Nursing diagnosis :

Impaired memory related to central nervous changes secondary to Alzheimers disease as evidenced by inability to recall recent events.

Goal or expected outcome :

Client may improve memory after implementation of nursing interventions.

Interventions and rationale :

* assess the motor function levels, it will give baseline data to plan nursing interventions.

* assess the degree of deficit, through this we can plan.

* assess emotional and psychological factors, because these may increases the dementia levels.

* recall that things when ever the time to remember, so that client can able to remember.

* provide adequate rest periods, so that we can replinise the client energy levels, memory levels.

* assist the client doing in self care needs, to promote well being of client.

* provide rehabilitation and occupational therapies, may improve the client memories.

Evaluation :

Client improved her rememberence capacit, after implementation of nursing interventions.
