Over the course of the next week, you see how these errors are impacting the entire revenue cycle from duplicate medical record numbers to wrong insurances listed.

2. How would you present this information to the patient registration director to reduce the number of errors?

There are various errors which arises in hospitals or clinics everyday and should be managed as soon as possible as it will affect the patient, staffs as well as the organisation itself.

1. The errors that is occuring from the patient registration in the hospital can be due to various reasons. Some of the reasons might be listed below:

- improper history collection and confusion in names and IP address

- lack of access to the medical history of the patient

- due to improper follow up with the patient after discharge

- implementing of the improper treatment for the patient.

2. The errors that has been occuring in the hospital should be informed or communicated to concerned person as soon as possible. I would present the errors to the patient registration director as soon as I confirm about the errors myself. I will inform him along with the adequate proof and data's collected from my investigations and with the possible reason behind these. I would also take few possible solutions along.
