I.Sentinel Event
A sentinel event is a patient safety event (not primarily
related to the natural course of the patient’s illness or
underlying condition) that reaches a patient and results in
any of the following:
– Death
– Permanent harm
– Severe temporary harm and intervention required to sustain
life.
An event is also considered sentinel if it is one of the
following:
– Suicide of any patient receiving care, treatment, and services in
a
staffed around-the clock care setting or within 72 hours of
discharge, including from the hospital’s emergency department
(ED)
– Unanticipated death of a full-term infant
– Discharge of an infant to the wrong family
– Abduction of any patient receiving care, treatment, and services
– Any elopement (that is, unauthorized departure) of a patient from
a staffed around the-clock care setting (including the ED), leading
to death, permanent harm,or severe temporary harm to the
patient.
– Hemolytic transfusion reaction involving administration of blood or
blood products having major blood group incompatibilities (ABO,
Rh, other blood groups)
– Rape, assault (leading to death, permanent harm, or severe
temporary harm), or homicide of any patient receiving
care,
treatment, and services while on site at the hospital
– Rape, assault (leading to death, permanent harm, or severe
temporary harm), or homicide of a staff member, licensed
independent practitioner, visitor, or vendor while on site at
the
hospital
– Invasive procedure, including surgery, on the wrong patient, at
the
wrong site, or that is the wrong (unintended) procedure.
– Unintended retention of a foreign object in a patient after
an
invasive procedure, including surgery
– Severe neonatal hyperbilirubinemia (bilirubin >30
milligrams/deciliter)
– Prolonged fluoroscopy with cumulative dose >1,500 rads to
a
single field or any delivery of radiotherapy to the wrong
body
region or >25% above the planned radiotherapy dose
– Fire, flame, or unanticipated smoke, heat, or flashes
occurring
during an episode of patient care
– Any intrapartum (related to the birth process) maternal
death
– Severe maternal morbidity.

An Interview With a Woman Living With Heart Disease: Mary Eriksmoen
Answers for all questions given in the question:-
Mary Eriksmoen, 53 years old
1. For her, it
started a little over a year ago.
When she would walk up thes she felt a pain in the middle upper
part of her chest. It radiated into her jaw. She later learned this
was called angina, and it meant her heart wasn't getting enough
blood. She was also getting headaches. The jaw pain was the worst,
though.
2.0 years earlier, her mom also had heart bypass surgery.
3. She used some anticoagulants..etc. And then , Mary had successful triple bypass heart surgery.
4. She've been
watching what she eat. The biggest struggle she's having since her
surgery is quitting smoking. She know she need to. She didn't smoke
for three to four weeks after the surgery, but shw just started
again and she shouldn't have.
She also went to cardiac rehab three times a week for 12 weeks. She
used exercise equipment, like recumbent cross trainers, stationary
bikes, and weights. While she exercised, her heart was being
monitored. She also spoke with a dietitian.
Her back limits her ability to get the exercise she need for a
healthier heart, so one of the things she do now is go to the
hydrotherapy pool at the hospital. She go about twice a week, and
it helps with both her back and heart. For other women out there
who can't exercise due to a disability of any kind, the
hydrotherapy is a good alternative. From her understanding, the
pool takes off a lot of the pressure on our joints and lets do
exercises that we can't do on land.
5. Just listen
to your body. For a long time, you've been felt that, as a female,
doctors would just say, “It's all in your head.” But one thing
you've learned is your body was telling you something was wrong. U
just needed to listen to it. If you think there is something wrong,
you should get help right away.
You also encourage women to pay attention to health public service
announcements on TV and the radio. Pay attention because it could
make the difference between life and death.
I think women need to understand the signs of heart disease. You
can have an issue with your heart even if you don't have the
crushing chest pain we often associate with a heart attack. Also, I
think it's important women understand that their heart attack
symptoms are different than men's, and if they experience any of
the symptoms, they should call 911.
