Peritoneal incision was extended longitudinally. Balfore self-retaining retractors were placed in the usual manner. The utero-vesical peritoneal reflection was incised transversely and the bladder flap was bluntly freed from the lower uterine segment. A low transverse uterine incision was made atraumatically, with modest uterine pressure and after the bladder blade was removed. The infant's head was delivered from footling breech presentation. The nose and mouth were suctioned with the DeLee suction trap prior to shoulder delivery. The delivered infant was a 3,883 gram female with Apgar scores of 9 at one minute and 9 at five minutes. After the umbilical cord was clamped and cut cord blood was obtained for evaluation. Cord gas was not sent. The placenta was manually removed, was intact and appeared normal.
