DESCRIPTION OF OPERATION: Under general anesthetic, the chest wall and arm were prepped and draped in the usual fashion. An 8 x 18 cm transverse ellipse of skin was outlined along the posterior bra line preoperatively and the lower skin incision was made to elevate a sub- Scarpa's flap inferiorly for around 10 cm. The upper incision was then made and the flap raised superiorly, identifying the upper border of the latissimus dorsi muscle and undermining deep muscle to free up the flap. Here the intercostal perforators were secured with Hemodips and the muscle was separated from the lumbodorsal fascia. An incision was made along the anterior chest wall mastectomy scar to create a subcutaneous tunnel. The flap was tunneled and delivered into the anterior chest wall. Closure of the back defect was completed with interrupted 0 Vicryl and Dermabond. The patient was repositioned in the supine position. The medial horizontal scar was then incised and undermined to accommodate the myocutaneous flap. The upper border of the right latissimus muscle was then anchored in place. Using a balloon sizer, we decided to use a 300 mL silicone gel implant. The sizer was removed. Hemostasis established and a #15 Blake drain placed and anchored. A 300 mL silicone gel permanent breast implant was placed in the breast pocket and tacked with a single stitch to prevent the implant from migrating laterally. The distal 3 cm of the flap was de-epithelialized and buried beneath the sternal skin. The skin was closed and suture lines were covered with Dermabond.
