PROCEDURE: The patient was prepared and draped in the normal sterile fashion, and injections were made with 1 percent lidocaine with 1:100,000 parts epinephrine into the external auditory canal wall. The tympanomeatal flap was elevated from 10 o'clock to 6 o'clock in a counterclockwise fashion, using a vertical rolling knife for incision and a duckbill for elevation. The middle ear was entered at 2 o'clock, the chorda tympani nerve identified, and the annulus lifted out of the tympanic sulcus. After elevating the tympanomeatal flap anteriorly, the ossicles were palpated. The malleus and incus moved freely, and the stapes was fixed. The posterior superior canal wall was curetted down after mobilizing the chorda tympani nerve, which was left intact. The stapes footplate was visualized easily and found to be markedly thickened. The pyramidal process was identified and the stapes tendon cut. An IS joint knife was used to dislocate the joint between the incus and stapes. Next, small and large Buckingham mirrors were used, along with a drill, to drill out the stapes footplate. After this was done, a 6 mm x 4 mm Schuknecht piston prosthesis was placed in position. Crimping was achieved, and there was an excellent fit. The stapes footplate area then was packed with small pieces of Gelfoam. The tympanomeatal flap was put back in proper position and the middle ear packed with rayon strips of Cortisporin with a cotton ball in the middle to form a rosette. The patient was awakened in the operating room and transferred to recovery in no apparent distress.
