Attention was directed over the 1st metatarsophalangeal joint, where a dorsal longitudinal incision was placed. This was deepened down to the neurovascular structures, which were retracted, and the joint capsule was entered dorsally and reflected off of the head of the 1st metatarsal and the base of the proximal phalanx. It was noted at the time that there were severe degenerative changes of the joint itself, which were consistent with X-ray findings. A sharp sagittal bone saw was then utilized to resect the head of the 1st metatarsal and base of the proximal phalanx, Both pieces then were removed. Utilizing appropriate Instrumentation and technique, a 2.0 short Swanson double-joint implant was placed in the surgical site. There was a good range of motion post Implantation. The area was copiously flushed with antibiotic Irrigation throughout the procedure. Then, the joint capsule was re-approximated and closed utilizing a series of eight simple interrupted 2-0 Vicryl sutures and the skin and subcutaneous tissues were re-approximated and closed utilizing 5-0 nylon continuous non-interrupting suturing. The patient was then draped toward the 2nd toe where a dorsal longitudinal incision was placed over the proximal interphalangeal joint, deepened down to the neurovascular structures, which were retracted. Joint capsule was entered transversely and reflected the extensor tendon complex dorsally off of the procimal phalanx and the base of the middle phalanx. The extensor tendon complex was re-sutured more proximally utilizing four 3-0 Vicryl sutures, and the area was irrigated throughout the procedure. The skin and subcutaneous tissues were re-approximated and closed utilizing a series of simple interrupted 5.0 nylon sutures and a horizontal mattress type suture. There was excellent alignment post closure of the second toe and the tourniquet was released before dressing. Circulation returned instantaneously to all digits of the left foot. A Betadine compressive dressing was then placed around the left forefoot. The patient was transferred from the OR to postoperative recovery room with all vital signs appearing normal
