PROCEDURE PERFORMED: Craniotomy and anterior 2/3 corpus callosotomy Use of neuronavigation, Brainlab Use of intraoperative microscope INDICATIONS: The patient is a 12-year-old with intractable epilepsy and drop seizures despite multiple antiepileptic drugs. The above procedure was indicated. Informed consent was obtained both verbally and in writing DESCRIPTION OF PROCEDURE: The patient was brought to the operating room. She was intubated and sedated in the usual fashion Intravenous lines were obtained by Anesthesia. The patient was then affixed to the Mayfield head holder, affixed to the table and placed in the supine position. All pressure points were padded. We then registered the patient's scalp fiducials to the Brainlab neuronavigation system. We then prepped and draped the patient in the usual fashion. We made a transverse incision just anterior to the coronal suture, crossing over the midline to the left side approximately 3 cm. We then dissected down to the skull. We then performed a craniotomy with 2 burt holes on the midline and 1 laterally on the right side. We then, using a curette, dissected away the dura from the skull. We then used the craniotome to perform a craniotomy flap, with the flap being 2/3 anterior to the coronal suture and 1/3 posterior. We then opened the dura in a U fashion and reflected the dura medially. We then brought in the microscope. We used the Greenberg retractor system to dissect down to the cingulate Eyrus. We then dissected the adhesions connecting the cingulate gyri together. We then saw the pericallosal arteries. We then put our retractors in between them. We then identified the midline and performed an anterior 2/3 corpus callosotomy under microscopic magnification. After we had satisfactory disconnection, we performed intraoperative MRI, which confirmed our disconnection. We then copiously irrigated the wound and closed the dura and placed DuraGen on top of this. We then replaced the bone flap and the bone dust in the edges of the bone flap. We then placed titanium plates and screws. We then closed the scalp in layers using 3-0 Vicryl sutures and running 4-0 Monocryl and removed the Mayfield head holder and wrapped the head with Kerlix_ Blank 1:
