Assessment consideration for this patient

There should be an Braille expert to assist for any writing part.

Specialised equipment as per government guidelines can be used to check for sensory and motor examination.

Instructions for the patient should be on Braille.

Small instructions can be given to her by signing on her hand. Her family can help in communication between the patient and nurse by interpretation of information.

For palpatory finding special non verbal instructions should given to her like if she feel pain she can raise finger .one finger 2 finger 3 finger on basis of severity

One should more rely on auscultatory finding and and investigation report .

Proper through head to toe examination should be performed in presence of a family member , She will feel more comfortable and some typical gesture and sign can be understood after interpretation from the family members.

Question b)

Appropriate nursing diagnosis is Impaired verbal and visual type non verbal communication related to visual and auditory impairment as evidence by absent of speech to verbal and visual response and inattention to light and sound.

Question c) Since the patient is deaf and blind and also having inadequate health literacy .The risk factors associated with this are

1)she will lack the skill to manage her health and prevent disease which is preventable which increase the possibility of morbidity.

2) She also report her health as poor and may lack health insurance.

3) she might not be able to manage chronic disease and very less likely to get hospitalized

4) Since there is also lack of clarity of communication with her due to her condition she might face difficulty in understanding health information .

5 ) Having inadeaquate health literacy decrease the chances of having improved health status.
