Assessment is the first step to rule out problems.. Mrs Carson has very dry skin and the vessels in her hands are very prominent.. Assess the skin turgor of the patient . Dry skin looks wrinkled and has presence of scaly appearance.

So during assessment, observe the appearance of skin. Very dry skin indicates dehydration and it makes the hand vessels more prominent..
