You have been hearing buzz in the news about how facilities are push towards a Community Health Record for greater Continuity of Care.

Depends on the population explosion in the United States,community health must be defined,developed and implemented.like electronic and personal health records community health records have both unique and overlapping information.Moreover,it includes data about community including social,physical and lifestyle determinants of health.this will give more complete view of population health and quality life of interventions in data driven and evidence based manner.through which organizations can develop systemic approach to improve community health.community health records also includes measures of community access to healthy food,crime rate,public transportation,utility and efficiency,economic factors related to health status.this is also a cost effective method as well as time saving.
