Moments before the technician arrives your nurse comes in with a puzzled look. You ask what's wrong and they mention that another EKG order has appeared for you. The nurse verifies that you have in fact just had one and steps out to contact the Cardiologist. As it turns out, the second EKG ordered by the Cardiologist was for another patient.

First, discuss the potential outcomes that could have occurred if the nurse hadn't been diligent in reviewing your orders. Describe the impacts this mistake may have had on you, the other patient, and the hospital?

Next - create a plan to prevent this mistake from happening in the future. Include in your plan what you think may have gone wrong within the Electronic Health Record. Be sure to comment on your peer's plans and how they might want to revise their plans.

Impacts of the mistake:

As a patient I would panic because a EKG had been performed .I would have thought that something serious has happened to me. This could detoriate my chest pain. The other patient would also suffer because his EKG would not be performed in time. This could be dangerous. If any mishap occurs this could affect the dignity and goodwill of hospital.

Plan for prevention:

The nursing staff that was with the cardiologist should be more attentive in taking orders and entering in electronic health record. Cross check should be done before telling the patient.The orders and patient's name should be matched before proceeding to carry out orders.
