Continuity of care document is the XML based markup that is aimed mainly at the encoding, structure and semantics of the coding of medical terms in patient summary and documentation exchange.
As a part of first part of the regulation set by US federal for incentives of electronic recording system that is known as meaningful use the continuity of care document and record both are chosen as examples for this clause. Meaningful care is justified by the aspect that in order to be permitted the continuity of care document should have preset records for diseases, lab reports, medications, allergies and patient infromation.
In stage two of meaningful use the continuity of care document was included as a part of standatds for clinical exchange of records. The second stage of meaningful care mandates the use of CCD by the clinician in exchange regulatory and care transitions.
