Client scenario :-

Client is an adult femae from Afghanistan .She is been diagnosed with Myasthenia gravis. As the treatment is very costly in outer countries and in our country it is cheap . So her parents planned to come to Fortis Escorts ( India ) for the treatment . It was my evening duty and I was posted the same the same patient . It was very difficult for me to communicate as they were not able to understand english ,to overcome this I used our translators service . She helped me to communicate with the patients . The second issue was that her hemoglobin level was low and the physician has ordered for for blood transfusions . It was the Ramzan time and they were not ready to donate blood . Respecting their culture beliefs , we took help from a voluntary blood donor and did the transfusions . The patients was having progressive mysthania gravis which were paralysing her limbs in ascending order and finally she had difficulty in breathing for that she was intubated because it was due to the progression of the disease . Feeding from the tube , IV fluids etc were started and a total of 1month she was bed ridden . At last plasmapheresis and IVIG were given for a couple of days which improved her condition and helped her to recover .
