Dose titration refers to gradual increase or decrease in the dose of medication administered to a patient

When a patent is getting discharge ,the titration dose is done generally to reduce and stop it.The patient is taught to decrease it in days.

For example

If a patient is on Percocet 25 mg and to taper down the following is adviced in taking it

25mg ×3 days

20mg×3 days

15mg×3 days

10mg×3 days

5mg×3 days and stop it.

Inform the patient that the medication will relieve pain and steady dose will be given before stopping to prevent any complications

Inform about the adverse reactions of the drug if not titrated as per physician order

Inform about drug dependence if drug dose is continued with the same dose.
