please help with the head to toe assessment of this patient(acute bacterial meningitis) under these categories. Thanks in advance.

1. Brian Jones, a 22-year-old college student, is admitted from the emergency department to a medical-surgical unit with the diagnosis of acute bacterial meningitis. The patient presented with the following vital signs: temperature, 104°F; blood pressure, 110/70 mm Hg; heart rate, 100 bpm; respiratory rate, 20 breaths/min. The patient presented with a severe, constant headache and nuchal rigidity.

General status: patient is a well-developed young man,but not alert and oriented.

Skin: warm and dry, no vascularity, no varicose veins., skin rash and petechiae is present.

Vital signs: Temperature-104F, Heart rate-100b/min, Respiration:20b/mt, BP-110/70mm Hg.

Head:Normocephalic, headache present.

Ears-pinna intact, in proper alignment, external canal patent, small amount of cerumen present

Nose-Patent bilaterally ;turbinates pink, no swelling.

Neck : nucchal rigidity is present.

Respiratory:lung fields clear, chest expansion symmetric.

Respiratory rate-20b/mt.

Cardiac:Rate:100bpm, tachycardia

Abdomen:No pulsation visible, rounded, active bowel sounds, no bruits or CVA tenderness, no palpable masses.

Extremities:A positive Kerning sign and Brudzinski sign.

Neurological system:altered mental status, residual neurologic dysfunction.
