Please help me ead to toe assessement of a patient that as a Pancreatitis, i wrote"The expected finding during head to toe assessment in the patient are, vomiting, fever and rapid pulse, dry mucous membrane, rigid abdomen, tachycardia, hypotension, bruising in the flank and around the umbilicus" but my instroctor those not want that she want something from

Condition pancreatitis :

General status : conscious, coherent, and looks dull because of pain, moderate body built.

Vital signs :

Temperature : 100 degree foreign heat.

Pulse : 102 beats per minute.

Respiration : 22 breaths per minute.

Blood pressure : 90 / 50 mm of hg.

Head : black color hair, no dandruff, no pediculosis ect.

Ears : good hearing capacity, clean and clear, visible tympanic membrane .

Eyes : normal vision, no invention or evertion of eye lashes, but pale conjunctiva, indicates anemia.

Mouth : dry mucous membrane in the mouth.

Nose : normal findings.

Throat : sore throat abserved.

Neck : nil significant of enlargement of lymph nodes, normal range of motion.

Respiratory system :

Pleural effusion and decreased oxygen levels observed.

Cardiac system :

Rapid heart rate observed, that is 102 beats per minute.

Abdomen :

Tenderness, bloating, nausea, vomiting, upper abdominal pain, these symptoms are worsen by eating.

Rigid abdomen, bruising in the flank and around the umbilicus.

Pulse : 102 beats per minute.

Extremities : normal.

Back or spine : upper abdominal pain radiates to back.

Skin : dryness due to dehydration because of nausea and vomiting, loss of skin tugur.

Neurological : weakness, fatigue, if severe pain leads to altered sensorium.
