Nursing Diagnosis

Ineffective breathing pattern related to constriction of bronchial tube or inflammation as evidenced by difficulty in breathing,increased respiratory rate,cyanosis

Outcome /Goal

Patient will be relieved from dyspnea and ensure a normal breathing pattern

Nursing intervention and Rationale

  • Assess the vital signs ,to rule out the baseline status and plan for care
  • Administer bronchodilator medication as per order ,to relieve spasm
  • Administer oxygen ,to relieve cyanosis by increasing the oxygen saturation
  • Provide Fowlers position ,this will increase the lung capacity
  • Assess paradoxical pulse ,during inspiration 12mmHg or greater systolic reading is a first sign of airway obstruction
  • Teach patient breathing exercise, to ease breathing

Evaluation

The patient should be able to breathe easily
