3. ANS: One of the main obstacles she might face would be her work schedule. Another one could be money.

4. ANS: As a nurse, we should be aware of her personal habits and what she likes and dislikes. When she goes home will she have a support system. We need to make sure before she leaves that we set her up with a plan. We should give her some handouts, website resources, and write down information for her to make sure she knows that we care about her health. Her care plan should make sure she follows up with the primary care doctor, knows how to take her medications, has resources to see a social worker if necessary. She needs to have some goals to make sure that she is following the proper diet and exercising like she should. We want her to be successful and stay healthy so she doesn’t put her health into danger.
