ANS. 1. Both Jose and Jorge were identical twins but separated by birth have different disease patterns due to social determinants. as they both were raised differently by two different families. Jose, on one hand, was raised in a slum community whereas Jorge, on the other hand, was brought up in a professional upper-middle-class family. due to the difference in lifestyle patterns, their health status was different as Jose did not have any medicare facility and was also not vaccinated at the time of childhood which resulted in developing measles at the age of 4. but Jorge was given proper medical facility and vaccinations, so he rarely got sick.

ANS. 2. The physical environment has a major effect on the health of Jose and Jorge. Jose was living in a slum community in very unhygienic conditions with more prone to diseases and infections. moreover, in his later life, he was exposed to crime and drugs which contracted him HIV with contaminated needles. on the other hand, Jorge was living a better life than Jose. he was brought up in a professional upper-middle-class family where he can access a good medical facility. in his school days, he developed asthma but with good treatment, he was able to play on sports teams.

ANS. 3. In the case of Jorge, where he can afford a good treatment of his illnesses, he managed to go for bypass surgery when he had a heart attack in his 40's and also he had good treatment for his problems for high cholesterol, high blood pressure, and diabetes. all these medicare facilities led him to lead a professional life.

while in the case of Jose, in his 30's he was contracted with HIV and then TB, for which he received treatment free of cost but later on he did not continue his treatment which costs him to lost 30 pounds (TB returned).

ANS. 4. Jose's health was getting worse day by day due to his negligence and other factors like the physical and social environment in which he was brought up. He has HIV and TB that led to the hospital staff to hospitalized him in the emergency to prevent others from this.
Jorge in his 50's developed progressive kidney disease but he managed for twice-a-week dialysis and was waiting for the kidney transplant. he also had good treatment for his high BP, cholesterol, and diabetes to lead a professional life.
