A clients prescription usually contains the name, age, sex and place the client is coming from. it also includes the present complaints and past medical history including the medications he or she taking. The details of diagnostic procedures if any will also be included and the treatment such as details of medication with there form, name( trade name), dose, route and frequency and for the number of days the medicine has to be taken is also included. it also specifies the next follow up and the days to take rest and other precautions to be taken.

the nurse should contact the provider to clarify if she feels doubt about any medications name, dose, route, and frequency to reduce errors. when the medications are given for any specific cause e.g hypertension, cardiac medicines we usually check vital signs prior to them, if on assessment the findings are normal or below normal we have to withhold the dose that is also to be clarified. if certain positions or comfortable devices are provided to the patient and the patient feels uncomfortable for them it is to be informed to the general practitioner. the patient who is not willing to follow the treatment, not cooperative to treatment should also be informed.
