1) Acute stress disorder adjustment disorder:

symptoms: severe anxiety, dissociation, poor concentration, hyper vigilance exaggerated startle response, motor restlessness.

The patient may feel difficult or impossible to experience pleasure in previously enjoyed activities, frequently feeling guilty about usual life tasks, difficulties in concentrating, illusions, flashback episode etc.

General treatment includes anti anxiety medication, selective serotonin reuptake inhibitors, cognitive behavioural therapy, hypnotherapy.

Nursing implications and nursing diagnosis: they may show symptoms like depression, phobic reactions. The psychological impact of any traumatic event affect people differently. For diagnosis the person must be experienced symptoms like reexperiencing, avoidance, persistant negative alterations in cognitions and mood, alternation in arousal and activities.

2) Depersonalisation disorder:

symptoms and patient feelings: patient do not able to judge between real and delusional world.

The disease can interfere with normal functioning and lead to distress.

General treatment: Psychotherapy or a talk therapy is recommended. The two paychotherapy involve cognitive behavioural therapy and psychodynamic therapy.

Nursing implications and diagnosis:

Note any signs for self mutilation or previous suicide attempt and setting limits for inappropriate behaviours. Get a baseline data and determine neurological status. There is no specific lab tests for this disorder. Nusing care plan and diagnosis include risk for self mutilation, low self esteem, impaired social interactions, and ineffective coping.
