Prior to the case the graft was prepared, looping the graft then whip-stitching the looped end and then keeping the two tails free. This was fit easily through and a little bit tight in the 6 mm sizer. This was then tensioned at 15 pounds for three hours. in was then opened down to the MPF ligament on the femoral side first. The guide pin was placed just the physis to make sure that the center was about 3 to 4 mm distal to the physis for the 6 mm tunnel. had good placement, this was then drilled across the medial femoral condyle, making sure not to enter ch. This was checked and then over-drilled with AP and lateral planes then drilled using a 6.5 mm reamer det AP plane, making sure we were distal to the physis which we were this point, a 2 to 3 cm longitudinal incision was made distal to the superior half of the medial aspect of natella, down onto the medial patellar surface was exposed along the superior half. This was then debrided het down to bleeding bone. At this point, two Bio-SutureTak double loaded anchors were placed, one at the midpoint and one about 7 to 8 mm proximal to this central in the bone, with position checked under fluoros- my prior to drilling. The button double-loaded anchor was then placed and noted to have good fixation. The palt was brought to the field and was passed from the midline out to medial passing the looped end. Once the graft was noted to be docked into the tunnel, the suture in the eyelet was then tied to itself and suture ends were cut. At this point, the free ends were then obtained. With the knee at 45 degrees of flexion, the patella was located so that the lateral edge of the patella was in line with the lateral femoral condyle. Then the superior suture was passed first, tensioned the graft to keep the patella in this position, riding along the lateral aspect of the femoral condyle. Then, in a horizontal mattress fashion the two tails were passed. They were tied down sequentially. This process was repeated again for the inferior anchor. The MPF incision was closed using 2-0 Vicryl and then a 3-0 Monocryl in a horizontal mattress fashion. Local anesthetic was injected throughout. wound was covered with Xeroform and then 4 x 8s, ABDs, soft roll and then the leg was placed into a TED ose and then a knee immobilizer with the knee locked in extension. The patient was awoken and extubated in stable condition to recovery.
