Answer #1

Ans:-

C,

Which colour of the drainage.

The nurse would ask the client about the colour of drainage on report of rhinorrhea during history taking of nose and sinuses because rhinorrhea may occur due to any infection, allergy, cold , crying and head injury and if it occurs due to head injury then it may be CSF that is coming from nose and which is clear in colour but if it due to cold then its colour will be cloudy and in case of infection it may be green in colour.
