The advanced registered nurses in the healthcare setting have placed an important role in patient safety and care throughout the time. But, their roles and responsibilities have transformed throughout the time under the influence of various technological, professional, government changes. Advanced registered nurses have also been under the influence of some changes from 20th century to 21st century in terms of patient expectations and shifts in scope. The earlier provider-based and free-for-service is transformed to team-based and greater patient-centered shifts. This made the nurses in the 20th century to change their approach of care to team work and patient centered care. Earlier the care provided by the nurses was independent and episodic. However, the transformation of American health sector has increased patient expectations from the nurses. This made the nurses to have greater coordination and effective communication skills.

The caring patterns of the advanced registered nurses will keep on evolving throughout the time due to the modifying pattern of the American healthcare system. The affordable care act of the United States has provided an approach of greater coordination of care across the healthcare providers. This has led to greater respect and recognition to the nurses. . The affordable care act developed new healthcare structure that required a number of health programs and initiatives. This has played an important role in the evolution of the roles and responsibilities of the registered nurses in terms of access to technology, knowledge and skills with a more patient-centered approach. Competencies such as change in the attitude, skills and knowledge can lead to an everlasting transformation in the nurses. Future of the American healthcare system demand the nurses to show high efforts toward patient satisfaction. This will require the nurses to gain competence in soft skills such as empathy, patience and effective communication. This can significantly indicate that the role and responsibilities of the advanced registered nurses will keep on changing from time to time
