Answer: Home health and related Community based system usually provide care to the patient at their own home. They takecare of the people who are unable to take self care as well as patient with physical disabilities etc. They provide better experience to the patient as they get care and trrtreatm at their own home. They don't need to shift from one place to another institution or heritage house.

They provide long term care to the elder people/ patient which makes them to feel ease as they get all the caring facilities at their home. These community based home health identify the risk of poor self management, environmental hazards, unable to recieve medications etc. and then they provide proper facilities and care to the patient.

In one of the survey it has been found that 90% of the elderly patients wants to stay and choose home health. The services provided at home health care shows much improvement in the Patients.
