1 ANSWER:

Readmission costs a lot to patient and family members as they are again going to admit in the same hospital or organization for failed treatment or procedures. As a nurse it is a key element to prevent readmission of patients to hospital stay. 24 hours nurse will take care of patient completely in a way to cure the disease conditions and discharge in a healthy way. Readmission occurs due to improper treatment plans, irrelevant course actions of medications, negligent behaviors of health team, unsupervised diagnostic findings and medical check ups. Nurses role is crucial in preventing readmission of patient. The role of nurses is as follows :

* Monitor for patients health condition, high risk cases, type of treatment, psychological , emotional balances at the time of admission

* Developing a healthy communication with patient and family members, which helps to develop a trust relation ship between them

* Promote teaching practices to patient in terms of hospital stay, treatment regimen, nutritional intake, life style modifications for early recovery may help to reduce readmission

* Provide complete health guide information on follow up visits after discharge to home

* Collect patient address, contact number with referrals to get in touch with patient for getting update information on health of patient

* Plan for a healthy interaction services with patient and family in clarifying of their doubts regarding the patients disease condition

* Improve knowledge on disease condition, its effects on health, re attack of the disease, prevention of condition, protection, healthy life style practices has to be educated in forms of audio visual aids, power point presentations, discussions, etc

Inter professional team is defined as getting cooperation, collaboration and coordination among team members of different health care professionals for providing patient centered care as a team work.

FOR EXAMPLE :

A clinician teacher prepare teaching to students of their own profession regarding clinical areas of work. At that particular point of time it will be difficult for the clinician to engage others, there coordinating of two people can be done- one by taking the student having experience in guiding faculty to run inter professional clinic and the other from inter professional experiences to students in hospital and community areas. By the coordination of these two interpersonal team learners can engage easily in the teaching plan and contribute their points for a effective teaching output.
