Brief description of the environmental pressure:
Falls rate in long-term care higher than senior living in the
community..Exposure to environmental hazards increases
approximately 27% in long term care..Environmental factors cause
this illness due to improper beds facilities,poor lighting,
slippery floors,lack of rest areas and lack of kick space under the
bed..Bedside is the most common location for falls,it happens
mostly during the day times,when health care providers are not
available staff shift change times..Women have higher rates of
fracture than men due to falls..
A description of the response carried out by the organization to
change course and adapt:
It is important that fracture prevention,interrelated health issues
in long-term care cause many injuries including death..Intervention
for this risk to prevent osteoporesis with vitamin D
supplementation decreased the risk of fractures, Biphosphonates to
decrease the risk of fracture by increasing bone density..There is
lot of psychological consequences due to fall like social
withdrawal,reduced quality of life etc..Fall management is an
important part of model of care..Effective intervention to prevent
falls including exercise,avoiding unsafe medication
practice,additional supplementary medicines reducing hazards in LTC
by avoiding poor lighting and cluster using hip
protectors,comprehensive fall risk prevention strategy includes
resident education and environmental
modification..Elderly individuals with physical disability not
using a walking aid avoid falls..Assistive device,immediate post
fall management,transferring with mechanical lifts,
monitoring,communication and documentation can improve the quality
of care..

It will work with the support of health care provider..they should
be aware of tendency for elderly individuals to dissociate
themselves from falling..It is responsibility for all the staffs
with communication following policies and procedures with
interdisciplinary team..

essential components in an overall strategy..Education about fall
risk,exercise program is the risk reduction strategy can improve
quality care and avoid risk and improve balance..supervise and

protectors,assistive devices,changes in position,bedside lock can
help understanding the complexity and eliminate fear of family and
residents..
